Fees & Insurance

I am currently in network with Pacific Source, MODA, OHP, United Health Care- NIKE plan only & Aetna. Many plans will pay in full or partially for out of network benefits. I make it very easy for clients wanting to access out of network benefits if you should choose.

Insurance plans vary widely in reimbursement rates/percentages. Clients are responsible for contacting their insurance ahead of time to inquire about benefits. At the time of your initial appointment I will collect the full fee and bill your insurance. After the first insurance payment is received, I will adjust your portion for each session based on what is paid and credit the difference toward future sessions.

I am able to accept all major credit cards. As a courtesy I will happily bill your insurance as an out-of-network provider. Payment in full is due at the time the service is rendered, credit cards will held on file.

Checking your Insurance Benefits

You are responsible for all fees not paid by your health insurance. As such, it is important to know what your health benefits are and what the out-of-pocket costs will be. I recommend that you contact your insurance company and ask the following: 

  • Is Fawn McCool, LCSW covered as a preferred provider or are her services out-of-network? You will need to use my address 9370 SW Greenburg Rd, Suite 321, Tigard, OR 97223. I may have been in-network at a previous address but no longer am.

  • What is my annual deductible and have I met it? Is my deductible waived or do I have to meet the deductible before my plan pays for counseling?

  • What is my co-pay or co-insurance for each session?

  • If Fawn McCool, LCSW is out-of-network, what percentage of her services does my plan cover? What percentage is my responsibility?

  • Are there limits on the number of outpatient mental health visits covered?

Private Pay

In order to use insurance for counseling you must meet criteria for a mental health diagnosis. The diagnosis is reported to your health insurance company and becomes part of your medical record. 

Some people choose not to use their health insurance for therapy. We can chat more about the benefits of self pay over the phone if you’d like.

Self-pay is an option if you do not have health insurance or do not want to use your health insurance for counseling.
